Nursing Care Coordinator

Position Overview
The role of a Registered Nurse (RN) involves delivering skilled nursing services to patients intermittently within their homes. This is executed in alignment with physician directives and the established care plan, under the guidance of the Clinical Manager.
Qualifications
- Must be a graduate from an accredited nursing program and hold a valid nursing license in the practicing state.
- Preferred: Minimum of one year of nursing experience.
- Alignment with the philosophy and objectives of Canyon Home Care & Hospice.
- Demonstrated ability to take initiative and make independent decisions.
Key Responsibilities
- Deliver nursing services as per the established care plan.
- Conduct initial evaluations to identify immediate care needs, assess eligibility, and develop care plans in collaboration with the interdisciplinary team.
- Regularly reassess patient needs and modify care plans accordingly.
- Initiate care plans and implement necessary adjustments.
- Provide specialized nursing services requiring advanced skills.
- Implement preventive and rehabilitative nursing measures.
- Document clinical notes and summaries of care promptly as per agency standards.
- Coordinate care with other team members, physicians, and caregivers.
- Communicate changes in patient conditions to relevant personnel.
- Support patients and families in addressing nursing-related needs.
- Engage in training and supervision of nursing staff.
- Adhere to established agency policies and procedures.
- Process physician orders and maintain communication regarding patient needs.
- Assess the type and extent of nursing care required for each patient.
- Refer patients to specialists as necessary.
- Supervise LPNs and home health aides according to regulatory requirements.
- Conduct patient care conferences and participate in quality management initiatives.
- Provide comprehensive patient care as needed.
- Participate in on-call duties as assigned.
- Complete necessary assessments and documentation in a timely manner.
- Maintain confidentiality and safeguard patient information.
- Attend staff meetings and care conferences as scheduled.
- Report any incidents or variances to the Clinical Manager.
- Exhibit proficient technical skills and adhere to agency standards.
- Maintain professional appearance and demonstrate effective time management.
- Ensure a clean and organized work environment.
- Exhibit sound judgment and critical thinking in decision-making.
- Maintain current CPR certification.
- Perform additional duties as assigned.
