Position Overview:

Role Responsibilities:

The Care Manager plays a pivotal role in the coordination, execution, and oversight of services delivered to clients, ensuring alignment with the organization's mission and care standards. This position is crucial for maintaining compliance with regulatory and agency standards, as well as ensuring the integrity of client records. This role requires periodic on-call availability.

Minimum Qualifications:

  • At least two years of experience in home care, home health, or a related field.
  • A dependable means of transportation, valid driver's license, and current insurance.
  • Exceptional written and verbal communication skills, with the ability to engage effectively with a diverse range of individuals including clients, families, care providers, and other professionals.
  • Strong attention to detail and follow-up skills, along with the ability to take initiative and solve problems effectively.
  • Capability to supervise, assess, and evaluate care providers while advocating for clients and their families.
  • A genuine passion for assisting others in maintaining their dignity.
  • A professional and positive business demeanor.
  • Successful completion of a criminal background check.

Key Duties and Responsibilities:

  • Conduct comprehensive Client Assessments for each new client and provide signed agreements to the client or their representative.
  • Develop and implement tailored care plans for each client, ensuring client and/or representative approval. Care plans should encompass:
    • Client's functional limitations;
    • Nutritional requirements and food allergies;
    • Necessary home medical equipment and supplies;
    • Type and frequency of services to be delivered;
    • Requested non-medical tasks.
  • Collect initial service fees during the assessment or on the first day of service.
  • Facilitate caregiver introductions for the first scheduled shift, reviewing care plans with both the client and caregiver to set clear expectations.
  • Oversee client care provided by agency personnel to ensure compliance with care plans and adherence to safety protocols.
  • Maintain regular communication with clients and/or their representatives to monitor service delivery and satisfaction.
  • Conduct scheduled and unscheduled in-home visits to support clients.
  • Perform reassessments as needed based on changes in client condition or circumstances.
  • Coordinate services when multiple community providers are involved in client care.
  • Act as a resource for clients and families, offering education, empowerment, and quality of life enhancements, along with necessary referrals.
  • Investigate and address any issues related to client care and well-being.
  • Document client care supervision accurately and maintain current client records in accordance with agency policies.
  • Provide training and education to staff in various settings.
  • Report relevant client and employee information to ensure effective service delivery and management.
  • Assist in the development and revision of client care policies and procedures.
  • Participate in caregiver evaluations.

Physical and Environmental Requirements:

Travel to clients' residences using reliable transportation is necessary.

Organizational Structure:

This position reports directly to the General Manager.
