Salary
$30 per hour

About the Role
This is a part-time position requiring night shifts. You will be responsible for implementing physicians' prescribed plans of care, conducting patient assessments, and administering medication and treatments as needed. Your attention to detail, organizational skills, and ability to work independently will be essential in this role.

Responsibilities
* Implement physician-ordered treatment plans for patients in their homes
* Conduct regular patient assessments to monitor progress and adjust care plans as needed
* Administer medication and treatments according to physician orders
* Maintain accurate and timely documentation of patient interactions and care provided
* Collaborate with multidisciplinary teams to ensure seamless patient care
* Participate in ongoing education and training to enhance clinical knowledge and skills

Requirements
* Current, unrestricted LPN/LVN license in the state of practice
* 6 months prior hands-on nursing experience preferred
* Reliable transportation and valid driver's license required
* Ability to lift up to 50 pounds and stand for extended periods
