Key Responsibilities:- Leadership and Management: Provide strategic leadership and direction to the hospice care team, ensuring that patient needs are met and staff are supported to deliver excellent care.
- Patient Care Coordination: Coordinate patient care plans, ensuring that patients receive the best possible care and that their needs are continually assessed and met.
- Staff Management: Manage and develop a team of RNs and other healthcare professionals, providing guidance, support, and training to ensure that they have the skills and knowledge needed to deliver high-quality care.
- Quality Assurance: Develop and implement quality assurance processes to ensure that patient care meets the highest standards, and that any issues or concerns are identified and addressed promptly.
- Communication and Collaboration: Communicate effectively with patients, families, and healthcare professionals to ensure that care is coordinated and that patients receive the best possible support.
- Compliance and Regulatory: Ensure that all aspects of patient care and staff management are compliant with relevant laws, regulations, and standards, including Medicare, Medicaid, and third-party payer requirements.
- Education and Qualifications: RN with a strong background in hospice care and leadership experience.
- Skills and Knowledge: Excellent leadership and management skills, with the ability to motivate and develop a team of healthcare professionals. Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Experience: Proven experience in hospice care leadership and management, with a strong understanding of relevant laws, regulations, and standards.
