Hospice Clinical Manager RN

We are seeking a highly skilled and experienced Hospice RN Clinical Manager to join our team at Career Strategies. As a key member of our leadership team, you will be responsible for overseeing patient and personnel assignments, coordinating patient care, and ensuring the development and implementation of individualized plans of care.
Key Responsibilities- Leadership and Management: Oversees patient and personnel assignments, coordinates patient care, and ensures the development and implementation of individualized plans of care.
- Patient Care Coordination: Coordinates referrals, assures patient needs are continually assessed, and develops, implements, and updates individualized plans of care.
- Staff Management: Increases or decreases staffing levels as applicable, conducts meetings and reviews patient care reports with appropriate personnel, and supports a culture of safety and quality practices.
- Communication and Collaboration: Communicates with physician's offices, payers, patients/families, outpatient services, and inpatient coordinators regarding patient care and status.
- Quality Assurance and Compliance: Participates in the Agency's information management and Quality Assurance Performance Improvement (QAPI) programs, possesses understanding of applicable state and federal regulations, and communicates updates to staff.
- Professional Development: Keeps up-to-date on new methods, products, trends, etc., and communicates this information to staff.
- Education and Experience: RN with experience in hospice or home health care management.
- Skills and Qualifications: Strong leadership and management skills, excellent communication and interpersonal skills, and ability to work in a fast-paced environment.
