Job Summary

Catholic Health Initiatives is seeking a skilled Dietitian to join our Diabetes Education team at our Midland's Family Clinic in Papillion. As a key member of our healthcare team, you will provide direct and indirect care to patients with diabetes, working collaboratively with healthcare professionals to ensure comprehensive care.

Responsibilities
  • Conduct thorough individualized assessments of patients with or at risk for diabetes.
  • Develop and implement individualized plans of care, integrating current diabetes care practices and established principles of teaching and learning.
  • Provide diabetes education and self-management training to patients, promoting healthy lifestyle choices and disease management.
  • Lead the development and maintenance of diabetic education program standards, policies, and procedures.
  • Participate in quality improvement activities related to patients with diabetes.
Requirements
  • Graduate of an accredited school of dietetics or nutrition.
  • Licensed as a registered dietitian in the state of Nebraska or Iowa.
  • Certified as a Diabetes Educator or Board Certified in Advanced Diabetes Management.
