Key Responsibilities
  • Develop and Implement Care Plans: Collaborate with patients, families, and healthcare teams to create personalized care plans that meet the unique needs of each individual.
  • Provide Skilled Nursing Care: Deliver high-quality nursing care to patients in their homes, including wound care, infusion therapy, and catheter management.
  • Coordinate Care Services: Work with patients, families, and healthcare teams to ensure seamless coordination of care services, including medication management and equipment setup.
  • Communicate Effectively: Maintain open and transparent communication with patients, families, and healthcare teams to ensure that care needs are met and concerns are addressed.
Requirements
  • Active RN License: Possess an active registered nurse license in the state of employment.
  • CPR Certification: Hold current CPR certification.
  • Valid Driver's License: Possess a valid driver's license.
Preferred Qualifications
  • Medicare Skilled Nursing Experience: Possess experience working in a Medicare skilled nursing environment.
  • Basic Understanding of Oasis: Have a basic understanding of the Oasis system.
  • 1-Year RN Experience: Possess at least one year of experience as a registered nurse in a healthcare setting.
