Job Summary

We are seeking a highly skilled Clinical Pharmacist to provide advanced clinical pharmacy services in a healthcare setting. This role involves working collaboratively with healthcare providers to ensure safe, effective, and appropriate use of medications in patient care.

About the Role

  • Provide clinical pharmacy services, including medication therapy management, drug information, and patient education, to patients and healthcare providers.
  • Collaborate with physicians, nurses, and other healthcare professionals to optimize medication therapy for patients.
  • Review and interpret patient medical histories, laboratory results, and other relevant information to determine appropriate medication therapy.
  • Monitor and assess patient medication therapy for effectiveness and adverse effects, making recommendations as necessary to healthcare providers.
  • Participate in medication safety initiatives, including identifying and reporting adverse drug events and medication errors.

Requirements

  • Doctor of Pharmacy degree from an accredited school of pharmacy.
  • Completion of a PGY-2 residency in a clinical pharmacy specialty or equivalent experience.
  • Board certification in a clinical pharmacy specialty (e.g., BCPS, BCCCP, BCOP, etc.) preferred.
  • Active pharmacy license within state of practice.
