Job Summary

We are seeking a skilled and compassionate Clinician to join our team in the Sub-Acute Stabilization program. As a key member of our interdisciplinary team, you will provide mental health services to individuals experiencing emotional distress or behavioral problems, including those with chronic mental illness, substance abuse problems, youth with autism and intellectual disabilities.

Key Responsibilities
  1. Conduct comprehensive clinical assessments and engagement with clients and their families.
  2. Determine DSM V Diagnosis in consultation with Supervisor and Medical Director or designated Psychiatrist, as needed.
  3. Develop treatment plans and contracts for services, implementing them as appropriate during the course of treatment.
  4. Discharge planning, which may include linkages to community support services.
  5. Provide case management services and maintain productivity targets determined by service.
  6. Participate and present in Peer Consultation and Quality Assurance Reviews.
  7. Foster collaboration with other clinical and administrative staff to respond to client and programmatic needs.
  8. Acquire and maintain the needed skills in evidenced-based models that the agency/program ascribes to.
  9. Partners with Supervisor to identify and develop plans for professional growth.
Requirements
  • MSW or equivalent required with a minimum of 3 years' post-graduate clinical experience preferred.
  • Clinical degree in social work, psychology, or other licensed-eligible counseling degree.
  • LMSW or equivalent required and licensed eligible.
