Position SummaryThe Nurse Clinician Coordinator embodies the mission and values of our organization, adhering to ethical standards and compliance protocols. This role is essential in delivering direct patient care and involves the comprehensive assessment, planning, implementation, and evaluation of care plans for assigned patients.
Key Responsibilities:- Deliver evidence-based patient care while being accountable for the assessment, planning, implementation, and evaluation of care plans.
- Facilitate timely access to care and coordinate complex care processes across the healthcare system.
- Oversee patient care throughout the continuum of services.
- Assist patients in understanding care recommendations as necessary.
- Evaluate the learning needs of patients and families, develop educational materials, and provide necessary education.
- Assess patients' physical and emotional recovery, promoting wellness and comfort.
- Act as a resource for patients and families, coordinating additional resources as required.
- Monitor the quality of care and patient satisfaction through follow-up assessments.
Required:
- Current Registered Nurse license in the State of Illinois.
- Bachelor of Science in Nursing (BSN) with a minimum of three years of clinical experience in ambulatory services.
- Proficient in computer applications, including Microsoft Office and database management.
- Demonstrated leadership capabilities and understanding of quality improvement processes.
- Ability to build clinical networks and lead systemic changes effectively.
- Competence in managing challenging situations and balancing multiple priorities.
- Exceptional verbal and written communication skills.
- Analytical skills for data-driven decision-making.
- Adaptability to changes in the work environment and unexpected events.
- Attention to detail with a focus on accuracy.
- Commitment to maintaining confidentiality.
Preferred:
- Three years of clinical experience in a specialized area.
- Experience in ambulatory operations.
- Background in process improvement initiatives.
