Home Health Licensed Practical Nurse (LPN) Job Description

At Interim HealthCare, we value our nurses and strive to create a work environment that supports their well-being and success. As a Home Health Licensed Practical Nurse (LPN), you will play a vital role in delivering high-quality patient care in the comfort of their own homes.

Key Responsibilities:
  • Provide home-based care to patients with illnesses, injuries, and chronic diseases, working under the supervision of a Registered Nurse (RN)
  • Collaborate with a team of healthcare professionals, including physicians, RNs, Certified Nursing Assistants (CNAs), and aides, to deliver comprehensive patient care
  • Take patient vitals, conduct assessments, document progress, and report changes to the RN supervisor
  • Assist the RN in implementing the patient's plan of care and teaching self-care techniques
  • Educate family members on the patient's plan of care, medication, and home safety
  • Work with the interdisciplinary team to coordinate patient care and plan for discharge
Requirements:
  • Graduate of an accredited practical nursing program and active LPN license in Colorado
  • Minimum of six (6) months of LPN experience, preferably in home healthcare, or completion of the office's preceptor program
  • CPR Certification (demonstration course required)
  • Knowledge of state and federal home health regulations
  • Clinical proficiency in nursing, strong communication skills, and compassion
