Home Health Licensed Practical Nurse

At Interim HealthCare, we're committed to delivering exceptional patient care in the comfort of their own homes. As a Home Health Licensed Practical Nurse, you'll play a vital role in our team's mission to provide compassionate and personalized care to our patients.

Key Responsibilities:
  • Provide high-quality, patient-centered care in the home setting, working closely with our interdisciplinary team to develop and implement individualized care plans.
  • Conduct thorough assessments, take vital signs, and document patient progress to ensure seamless communication with our team and patients' families.
  • Collaborate with our team of physicians, RNs, CNAs, and aides to deliver comprehensive care, addressing patients' physical, emotional, and social needs.
  • Educate patients and their families on self-care techniques, medication management, and home safety to promote independence and confidence.
  • Participate in ongoing education and training to stay up-to-date on the latest best practices and industry developments.
Requirements:
  • Graduate of an accredited practical nursing program and active LPN license in Colorado.
  • Minimum of six (6) months of LPN experience, ideally in home healthcare, or completion of our office's preceptor program.
  • CPR Certification (demonstration course required).
  • Knowledge of state and federal home health regulations.
  • Clinical proficiency in nursing, strong communication skills, and compassion.
