Overview of the Role

Matrix Medical Network is a leading provider of clinical services, offering a range of solutions to support primary care providers and at-risk health plan members. As a Clinical Health Specialist, you will play a vital role in our team, conducting comprehensive health assessments and providing expert care to patients in their homes, skilled facilities, or through telehealth visits.

Compensation Package

We offer a competitive compensation package for this position, including an estimated salary of $105 per completed assessment. In addition to your base pay, you may also be eligible for mileage reimbursement and other benefits.

About the Position

This is a part-time opportunity with flexible scheduling, allowing you to balance your work and personal life. As a Clinical Health Specialist, you will have the autonomy to work independently, using your expertise to assess patient needs and develop personalized care plans. Your strong communication skills and ability to work collaboratively with healthcare professionals will be essential in this role.

Responsibilities
  • Conduct comprehensive health assessments, including reviewing medical history, observing patient behavior, and developing treatment plans
  • Work closely with physicians, case managers, social workers, and other healthcare professionals to ensure seamless patient care
  • Collaborate with patients and their families to educate them on disease management, medication adherence, and healthy lifestyle choices
  • Provide follow-up care and document patient progress according to Matrix guidelines and protocols
Requirements
  • A Master's degree in Nursing or a related field, or equivalent experience and licensure as a Nurse Practitioner
  • Current RN and NP licensure in the state of practice, with prescription authority or the ability to obtain it
  • Board certification by the AANP, ANCC, or AACCN in a Matrix-approved specialty
  • A minimum of 1 year of experience as a Nurse Practitioner, with a strong background in gerontology and adult health
  • Excellent verbal and written communication skills, with the ability to work effectively with diverse populations
