Job Responsibilities:
  • Deliver Post-Acute Care: Provide high-quality care to patients in post-acute settings, including hospitals, rehabilitation centers, and skilled nursing facilities.
  • Monitor Vital Signs: Track and record patients' vital signs, including temperature, blood pressure, and oxygen saturation.
  • Administer Medications: Properly administer and verify medications, ensuring patient safety and well-being.
  • Contribute to a Safe Environment: Help maintain a safe and comfortable environment for patients, including proper infection control and fall prevention.
Qualifications and Skills:
  • Current LPN License: Hold a current Massachusetts state license to practice as a Licensed Practical Nurse (LPN).
  • Education and Experience: Completion of an approved LPN educational program and a minimum of 6 months of experience in a related role.
  • Physical Demands: Ability to lift a minimum of 50 pounds and perform other physical demands of the job.
