We are seeking a skilled Advanced Practice Clinician to join our team at Optum Home & Community Care. As a key member of our healthcare team, you will play a vital role in providing high-quality patient care and ensuring a smooth transition from rehab to home.
Key Responsibilities- Conduct comprehensive assessments and document findings in a concise and compliant manner.
- Ensure accurate and appropriate documentation of diagnoses and quality elements.
- Perform initial medication reviews and communicate with patients and team members as needed.
- Travel between care sites and participate in care coordination activities.
- Certified Nurse Practitioner or Physician Assistant through a national board.
- Graduate of an accredited master's degree in nursing or physician assistant degree program.
- Active and unrestricted license in the state of practice.
- Current active DEA licensure or ability to obtain post-hire.
- Access to reliable transportation and ability to navigate stairs and varying dwelling conditions.
- 1+ years of hands-on post-grad experience in Long Term Care.
- Understanding of Geriatrics and Chronic Illness.
- Proficient computer skills and ability to document medical information.
