Affinity Hospice is seeking a dedicated and compassionate Registered Nurse to join our team in a 7 day on/7 day off, On Call position. As a key member of our interdisciplinary team, you will provide exceptional care to our patients and their families, while promoting a culture of excellence and compassion.
Key Responsibilities- Case Management: Collaborate with the interdisciplinary team to develop and implement patient care plans, ensuring seamless coordination of services and optimal patient outcomes.
- Clinical Direction: Provide clinical guidance and support to the team, ensuring that patients receive high-quality, evidence-based care.
- Infection Control: Implement and maintain infection control measures to protect patients, staff, and the community.
- Quality Assurance: Monitor and evaluate patient care to ensure compliance with regulatory requirements and Affinity Hospice's standards of excellence.
- On-Call Responsibilities: Provide on-call service to patients and families, responding to urgent needs and concerns outside of regular business hours.
- Education: Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
- Licensure: Current State license as a Registered Nurse and current Driver's License.
- Experience: Two years of experience as a Registered Nurse in a clinical care setting, home health, or hospice preferred.
- Skills: Nursing skills as defined by generally accepted standards of practice, good interpersonal skills, and proof of CPR, Hepatitis consent/declination, and Covid Vaccination or eligibility for a religious/medical exemption.
- Transportation: Reliable transportation and valid and current auto liability insurance.
