Quadrant Inc is seeking a skilled Social Worker to join our team in Washington, DC.

Job Summary:

We are looking for a compassionate and experienced Social Worker to provide high-quality patient care and support in our inpatient setting. The ideal candidate will have a Master's degree in social work and at least two years of experience working as a Social Worker, with a strong background in building relationships with patients and families.

Key Responsibilities:
  • Collaborate with healthcare teams to complete referrals to community agencies and provide timely and accurate documentation.
  • Conduct psychosocial histories and socioeconomic assessments to identify patient needs and develop comprehensive care plans.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
  • Develop and maintain information on community resources and referral requirements to support patient needs.
  • Participate in multidisciplinary care coordination rounds and contribute to quality and service improvement initiatives.
Requirements:
  • Master's degree in social work from a Council on Social Work Education-accredited program.
  • At least two years of experience working as a Social Worker, with a strong background in hospital settings.
  • LGSW, LCSW-C, or MSW certification required.
