Job Overview

We are seeking a skilled Home Health Licensed Vocational Nurse to join our team in the Fort Worth, TX area. As a Home Health Nurse, you will provide high-quality patient care and work closely with our RN Case Managers to develop and implement individualized care plans.

Key Responsibilities:

  • Skilled Visits
  • Assist in the development, review, and revision of the plan of care
  • Provide quality patient care as set forth by the RN Case Manager's Care Plan
  • Observe, record, and report any changes in the client's condition to appropriate personnel and/or physician
  • Provide ongoing instruction and supervision of the client, family, or other health team personnel in the plan of treatment

Requirements:

  • Geriatric Home Health Experience
  • Texas Nursing License
  • Strong organizational and self-management skills
  • Strong and compassionate customer service skills
  • Valid driver's license
