Job SummaryWe are seeking a skilled Primary Care Physician to join our team as a Medical Director. In this role, you will be responsible for leading our primary care and wellness initiatives, working closely with our team to develop and implement population health management strategies.
Responsibilities- Lead our primary care and wellness initiatives, developing and implementing population health management strategies
- Collaborate with our team to design and implement care management programs
- Work closely with our health plan partners to ensure seamless care coordination
- Develop and maintain relationships with key stakeholders, including patients, families, and community partners
- Stay up-to-date on industry trends and best practices in population health management
- MD or DO degree
- Board Eligible or Board Certified in Family or Internal Medicine
- Current DEA license
- Florida licensed physician or eligible to obtain FL license
