About the Role

We are seeking a dedicated Nurse Care Coordinator to join our team at SSM Health, supporting our mission to provide exceptional patient care and make a positive impact on the communities we serve.

Job Summary

This part-time RN-Case Manager position is responsible for ensuring patients are discharged safely from the hospital, identifying potential barriers, and collaborating with the care team to address these challenges. The successful candidate will possess excellent communication skills, a strong understanding of healthcare regulations, and the ability to work effectively in a fast-paced environment.

Key Responsibilities
  • Assess patients' discharge planning needs and develop individualized plans to ensure safe transitions to the next level of care.
  • Collaborate with an interdisciplinary team to identify and mitigate potential barriers to discharge, including social determinants of health and complex medical needs.
  • Utilize evidence-based practice and scientific knowledge to inform nursing care, prioritizing patients' age-specific needs and clinical requirements.
Requirements
  • Graduate of an accredited school of nursing or equivalent education licensure.
  • Three years' experience in acute care nursing, with a focus on discharge planning and case management.
  • Registered Professional Nurse (RN) license in Illinois, Missouri, or Oklahoma.
