We are seeking a compassionate and skilled LPN to join our team at Three Pillars Senior Living Communities. As an LPN, you will play a vital role in providing high-quality care to our residents, ensuring their physical, social, and emotional needs are met.
Key Responsibilities:- Administer medications and treatments as ordered by physicians and in accordance with accepted standards of practice and Three Pillars policies and procedures.
- Maintain accurate and timely written and electronic charts/documents/records as required by law, regulations, and Three Pillars policies and procedures.
- Observe and record resident condition information, reporting findings to and collaborating with RNs to develop and implement plans of care.
- Communicate resident change of condition to appropriate persons, including nurse managers, RNs, physicians, family members, and other healthcare professionals.
- Assist in the admission/readmission process, observing and gathering clinical data, and developing and implementing initial plans of care.
- Obtain, transcribe, and implement physician orders according to accepted standards of practice and Three Pillars policies and procedures.
- Support and participate in all resident care programs, including bowel and bladder, skin integrity, pain, and other programs, following individual plans of care, documenting observations, and offering feedback and suggestions.
- Minimum education, certification, and/or licensure: Licensed Practical Nurse in Wisconsin.
- Current and valid CPR certification.
- Experience in long-term care preferred.
- Basic computer knowledge.
- Able to frequently push/pull objects weighing 60 pounds and occasionally lift and move objects weighing 100 pounds or more.
