Job Title: Nurse Navigator - Women's Health

UPMC is seeking a skilled Nurse Navigator to join our team at the Magee Womens Pregnancy and Women's Recovery Center at UPMC Magee Womens Hamot Hospital in Erie, PA.

About the Role:

The Nurse Navigator will work closely with our interdisciplinary team to provide comprehensive, coordinated, and compassionate care to women with substance use disorders. This includes utilizing harm reduction principles, developing wholistic plans of care, and empowering women to become leaders in their own recoveries.

Responsibilities:
  • Collaborate with the interdisciplinary team to guide patients and families in diagnostic and treatment options.
  • Provide emotional support, specialized patient care, support, and educational needs.
  • Assume the role of care coordinator and patient advocate throughout the process.
  • Use the nursing process to provide comprehensive nursing care, support, and education to patients and their families.
  • Provide expert knowledge of specialty issues to healthcare professionals and patients/families.
  • Work closely with the interdisciplinary team to meet the education and development needs of professional and ancillary staff.
  • Participate in the evaluation of patient care and programs through quality improvement activities.
  • Monitor changes in legislation and regulations to ensure effective actions are taken in a timely and consistent manner.
  • Assess and address the educational needs of individual patients and members of the community.
  • Provide staff development and serve as a clinical resource and consultant to members of the interdisciplinary team.
  • Coordinate tests, treatments, referrals, and appointments as necessary to meet the physical, spiritual, and psycho-social needs of patients and families.
Requirements:
  • Registered Nurse currently licensed in the practicing state.
  • Current certification in specialty or obtained within one year.
  • CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire.
  • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR).
  • Registered Nurse (RN).
  • Act 34.
