Job Title: Flex Full-Time Nurse Navigator

UPMC is seeking a skilled and compassionate Nurse Navigator to join our team at the Magee Womens Pregnancy and Women's Recovery Center at UPMC Magee Womens Hamot Hospital in Erie, PA.

About the Role:

The Nurse Navigator will work closely with our interdisciplinary team to provide comprehensive, coordinated, and compassionate care to women with substance use disorders. This includes utilizing harm reduction principles to develop wholistic plans of care and empowering women to become leaders in their own recoveries.

Responsibilities:
  • Collaborate with the interdisciplinary team to guide patients and families in diagnostic and treatment options, providing emotional support and specialized patient care.
  • Use the nursing process to provide comprehensive nursing care, support, and education to patients and their families during screening and diagnostic tests, treatment, recovery, and rehabilitation.
  • Provide expert knowledge of specialty issues to healthcare professionals and patients/families, working closely with the interdisciplinary team to meet education and development needs.
  • Integrate current research findings and technology into practice, performance improvement, and education as appropriate.
  • Participate in the evaluation of patient care and programs through quality improvement activities, identifying and coordinating performance improvement activities.
  • Monitor changes in legislation and regulations to ensure effective actions are taken in a timely and consistent manner.
  • Assess and address the educational needs of individual patients and members of the community, providing individualized teaching based on needs.
  • Provide staff development and serve as a clinical resource and consultant to members of the interdisciplinary team, identifying additional resources as needed.
  • Coordinate tests, treatments, referrals, and appointments as necessary to meet the physical, spiritual, and psycho-social needs of patients and families.
Qualifications:
  • RN with Bachelor's Degree preferred.
  • Three to five years of current clinical experience required.

Preferred experience with the population served (i.e. women's health, breast health, oncology, etc.) is a plus. Must be self-directed and possess strong leadership and communication skills.

Licensure, Certifications, and Clearances:
  • Registered Nurse currently licensed in the practicing state.
  • Current certification in specialty or obtained within one year.
  • CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire.
