Job Summary

The State of New York is seeking a highly skilled Registered Nurse 2 Community to join our team. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and support to individuals with mental illness, developmental disabilities, addiction, or substance use disorders in a community setting.

Key Responsibilities
  • Work within databases and systems to review and verify eligibility determination for individuals referred for PASRR Level II assessment.
  • Guide referral sources to the process of eligibility determination and advise on necessary assessments.
  • Collaborate with Front Door staff to ensure eligibility review continues while PASRR assessment occurs.
  • Communicate with hospitals and nursing facilities regarding referral package requirements and specific nursing facility needs.
  • Determine eligibility classification and complete Level II PASRR assessment.
  • Assure communication with regional office staff on admission and discharge status of individuals in nursing facilities.
  • Provide in-person or virtual assessments and review needs in assigned geographic areas.
  • Provide continued monitoring and advocacy with internal and external supports.
  • Arrange meetings and collaborations between involved entities to ensure best outcomes for individuals.
Requirements
  • Must maintain a valid New York State license and registration.
  • Currently a permanent Registered Nurse 2 (Community) or eligible to transfer via Section 70.1 of the Civil Service Law.
  • Open to public: licensed and currently registered to practice nursing in New York State and two years of post-licensure clinical nursing experience.
