We are seeking a highly skilled Medical Assistant/Medical Scribe to join our team as a Home-Based Care Specialist.

About the Role

The Home-Based Care Specialist will play a crucial role in supporting logistics, clinical care, and documentation in our innovative home-based care program. You will work directly with a House Calls nurse practitioner on a daily basis, in collaboration with center-based care teams, to help identify and bridge gaps in care when patients return from the hospital or experience other challenges.

Key Responsibilities
  • Prepare visit schedules, routes, and logistics for all visits
  • Accompany nurse practitioner on all house calls
  • Take vital signs, reconcile medications, and educate patients and families
  • Document House Call notes, manage referrals, orders, and follow-up
  • Maintain supplies and equipment
What We're Looking For

We're seeking a highly motivated and detail-oriented individual who is passionate about primary care and reaching vulnerable patients. You should be comfortable navigating visits in patient homes and have excellent communication, follow-up, teamwork, and problem-solving skills.

Requirements
  • At least one year of experience as a Medical Assistant with MA certification
  • Must pass all clinical competencies before supporting patient care
  • Daily access to a vehicle with valid license and insurance
  • Flexibility to travel around Chicagoland
  • Bilingual (English and Spanish) a plus
