We are seeking a skilled RN Patient Care Navigator to join our team at North Shore Inc. As a key member of our healthcare delivery system, you will play a vital role in ensuring seamless care coordination for our patients.
Key Responsibilities- Connect with patients at the time of diagnosis and follow them across the hospital stay and 90 days post-discharge, guiding them through the health system and eliminating barriers to access.
- Partner with the healthcare team to ensure timely and appropriate clinical decision-making, implementation of recommendations, and discharge planning.
- Facilitate payor authorization processes and ensure accurate patient type assignment.
- Collaborate with the CDI team to ensure accurate documentation reflecting severity of illness and intensity of service.
- Bachelor's degree in Nursing from an NLN-accredited school, with 2+ years of clinical nursing experience and a minimum of 2 years of utilization review, discharge planning, case management, or disease management experience.
- Knowledge of InterQual or MCG criteria, with clinical certification and experience with Microsoft Office Suite, Electronic Medical Records, and strong interpersonal and oral communication skills.
- Proven leadership skills and ability to work independently, setting priorities to coordinate care plans efficiently.
