We are seeking a skilled and compassionate Registered Nurse to join our Home Health team as a Weekend RN. As a Home Health Registered Nurse, you will play a vital role in providing high-quality patient care and support to individuals in their homes.
Key Responsibilities- Provide admission, case management, and follow-up skilled nursing visits for home health patients.
- Confer with physicians to develop and revise patient care plans.
- Implement patient care plans in conjunction with patients and families to achieve optimal resolution of needs and problems.
- Coordinate and oversee the work of Home Health Aides and Personal Care Workers.
- Monitor and document patient care and progress.
- Diploma, Associate, or Bachelor Degree in Nursing.
- Minimum of one year of nursing experience, preferably in med-surg, ICU, ER, or acute care.
- Current and unrestricted Registered Nurse licensure.
- Current CPR certification.
- Strong organizational and communication skills.
