We are seeking a highly skilled and detail-oriented Medical Care Coordinator to join our team at Think Whole Person Healthcare. As a key member of our healthcare team, you will play a vital role in coordinating and implementing care plans for patients under the direct supervision of a health care provider.

In this role, you will work closely with members of the healthcare team to coordinate patient care services, including preparation of patients for examination or treatment, gathering of information, and documentation of findings in the electronic health record (EHR). You will also ensure accurate documentation is maintained, relay instructions to patients, schedule follow-up appointments, and help ensure timely patient flow.

As a Medical Care Coordinator, you will be responsible for:

  • Performing vital signs, height, weight, and screening procedures and documenting findings in the EHR
  • Ensuring accurate documentation is maintained, relaying instructions to patients, scheduling follow-up appointments, and helping ensure timely patient flow
  • Collecting and/or assisting providers in collection of specimens, as required, using appropriate protocols
  • Assisting with special procedures and examinations following appropriate protocols such as infection control, universal protocol, and patient identification
  • Maintaining organization and cleanliness of examination rooms; stocking assigned rooms/cupboards and ensuring par levels of supplies are maintained

To be successful in this role, you will need:

  • A high school diploma or equivalent
  • Ability to achieve certification as a Medical Assistant (CMA, RMA, etc.)
  • An active medication aid and American Heart Associate BLS/CPR certification
  • One (1) year of experience as a Medical Assistant, utilizing electronic Health Records (EHR)
