Key Responsibilities:

  • Provide clinical teaching and competency validation for caregivers and nursing staff.
  • Ensure compliance with company policies, state licensure, and regulations, as well as state reimbursement regulations.
  • Facilitate communication and serve as a liaison between nursing, caregivers, family, and community medical providers.
  • Directly oversee initial start of healthcare, assessment, and first home visit with ongoing performance monitoring.
  • Directly supervise the implementation, coordination, maintenance, evaluation, and termination of care plans and services.
  • Market to healthcare providers in the service area as requested.
  • Provide skilled nursing care and treatment to clients per physician orders and plan of care.
  • Accurately document nursing actions, individual health status, and communicate with family and physicians.
  • Document progress and outcomes for established goals and inform physician, case manager, and families of changes in client medical condition and needs.
  • Provide health teaching to clients and assist in learning appropriate self-care techniques and safety measures.
  • Conduct nursing assessment visits as needed.
  • Report all incidents per operation procedure and regulatory standards.
  • Follow policies and procedures according to operation, state, and regulatory standards.
  • May be asked to cover after hours, weekends, and holidays on call to meet client needs.

Requirements:

  • Bachelor's Degree in Nursing, Social Services, or related healthcare field of study.
  • Healthcare management experience with multiple age groups.
  • Minimum of two years of clinical experience and one year in related management/supervisory role in Home Care and/or Home Health.
  • Current and unrestricted registration and license in the state of operation.
  • Maintain state regulatory certifications as required by state/program requirements.
  • General knowledge of computers and ability to learn industry-specific software and documentation applications.
  • Capable of working responsibly with highly confidential information.
