LTSS Service Coordinator

We are seeking a highly skilled Service Coordinator to join our team at Elevance Health. As a Service Coordinator, you will play a critical role in providing support to individuals with long-term services and supports (LTSS) needs.
Key Responsibilities- Conduct face-to-face assessments with individuals to identify their LTSS needs and develop personalized support plans.
- Collaborate with interdisciplinary teams to ensure comprehensive care coordination and support.
- Manage non-clinical needs of members with chronic illnesses, co-morbidities, and/or disabilities.
- Document member goals and progress in collaboration with the care team.
- Identify members who may benefit from alternative levels of service or other waiver programs.
- Participate in process improvement initiatives and serve as a subject matter expert for new staff.
- Bachelor's degree in a healthcare-related field or equivalent experience.
- Minimum of 2 years of experience working with a social work agency or equivalent experience.
- Experience working with individuals with LTSS needs.
- Knowledge of Medicaid and waiver programs.
