We are seeking a highly skilled and experienced Nurse Care Coordinator to join our Pediatric Gastroenterology team at Pediatric Specialists of Virginia. The successful candidate will be responsible for coordinating care for our outpatient gastroenterology population, working closely with our physicians and healthcare team to ensure seamless patient care.
Key Responsibilities- Administrative Functions:
- Coordinate patient care from diagnosis to treatment, including disease-specific education and symptom management.
- Interact with patients during office visits and via phone, assessing patient needs and triaging calls to the healthcare team.
- Document all interactions, care provided, and findings in the medical record.
- Utilize order entry for diagnostic testing, prescription refills, and other patient instructions requiring provider signature.
- Follow up on insurance authorizations and communicate with other departments and institutions to clarify questions related to patient care.
- Provide additional support to providers, including tracking results of laboratory, radiological, and pathological studies, and following up with patients as needed.
- Serve as a clinical resource for staff, patients, and caregivers, and coordinate with clinic staff to ensure a positive patient experience.
- Clinical Functions:
- Perform procedures such as replacing G Tubes and NG Tubes.
- Provide education and teaching to patients and caregivers.
- Administer medications as needed.
- Graduation from an accredited school of nursing.
- Valid licensure in the state of Virginia as a Registered Nurse (RN) or ability to obtain license in Virginia.
- One (1) year of pediatric experience.
- Five (5) years of RN work experience.
- Valid BLS/CPR certification.
- BSN preferred.
