Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at IntelyCare. As an LPN, you will have the opportunity to work in post-acute facilities in the area, providing high-quality patient care and contributing to a safe and comfortable environment.

Key Responsibilities
  • Provide direct patient care, including administering medications, monitoring vital signs, and documenting health concerns and medical history
  • Conduct intakes, review resident charts and medical records, and participate in IV set up, immunizations, and phlebotomy procedures
  • Assist with the cultivation of a safe, comfortable facility, and maintain appropriate stocks of medical supplies
  • Participate in the orientation and training of medical staff, and help to ensure a smooth transition of care for residents
Requirements
  • Current Rhode Island state license to practice as a Licensed Practical Nurse (LPN)
  • Completion of an approved Licensed Practical Nurse (LPN) educational program
  • High school graduation, GED, or equivalent
  • At least 6 months of experience (LPN or similar role)
  • Ability to lift a minimum of 50 lbs
