Key Responsibilities

The Licensed Practical Nurse (LPN) in Home Health is tasked with delivering exceptional patient care and executing technical nursing procedures in the comfort of the patient's home.
  • Administers care within the legal scope of practice as outlined by state regulations, adhering to the established care plan, and collaborating with the healthcare team. Modification of the care plan is not permitted.
  • Educates the patient's family on care techniques and the maintenance of a safe and healthy living environment.
  • Monitors, documents, and communicates with the supervising nurse and/or physician regarding the patient's treatment responses and any changes in their condition on a daily basis.

Qualifications

Licensure Requirements:
  • Active LPN license in Rhode Island
  • Valid Driver's License, vehicle insurance, and reliable transportation or access to public transit.
  • Current CPR certification is mandatory.
