We are seeking a skilled Registered Nurse to join our team at Elite Home Health, a part of LHC Group. As a Registered Nurse, you will provide high-quality patient care in the comfort of their own homes.
Responsibilities- Conduct comprehensive assessments of patients to develop individualized care plans.
- Coordinate care with other healthcare professionals to ensure continuity of patient care.
- Provide education and support to patients and their families to promote health and well-being.
- Collaborate with physicians and other healthcare professionals to ensure accurate and timely documentation.
- Ensure patients meet home health eligibility and medical necessity guidelines.
- Current RN licensure in the state of practice.
- Current CPR certification.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
