We are seeking a skilled and compassionate Registered Nurse to join our team at Elite Home Health. As a Registered Nurse, you will play a vital role in providing high-quality patient care in the comfort of their own homes.
About the Role:- Provide comprehensive nursing care to patients in their homes, as prescribed by physicians and in compliance with applicable laws and regulations.
- Coordinate total plan of care with other healthcare professionals involved in patient care and ensure continuity of care by planning and exchanging information with physicians, agency personnel, patients, families, and community resources.
- Make initial and comprehensive nursing evaluation visits, determine primary focus of care, develop plans of care within state-specific guidelines, and submit accurate, complete, and timely documentation in collaboration with physician orders.
- Ensure patients meet home health eligibility and medical necessity guidelines as defined.
- Current RN licensure in the state of practice.
- Current CPR certification.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
