Job Description

We are seeking a Registered Nurse or Registered Dietitian to join our diabetes management team. The ideal candidate will have experience working with patients with diabetes and a strong background in education and support. As a member of our team, you will work closely with patients, families, and healthcare providers to develop personalized plans of care that address the physical, emotional, and educational needs of patients with diabetes.

The successful candidate will have excellent communication and interpersonal skills, with the ability to work effectively with diverse populations. Bilingual (Spanish) language skills are highly desirable. Experience working in an ambulatory setting and proficiency in electronic medical records is also preferred.

Responsibilities
  • Provides comprehensive patient assessments, including collection of relevant medical data and development of individualized plans of care.
  • Educates patients and families on self-management techniques and provides ongoing support and follow-up care.
  • Liaises with multidisciplinary teams to optimize patient outcomes and coordinate care.
  • Serves as a resource for patients, families, and healthcare providers, providing education and guidance on diabetes management.
Requirements

Education: Bachelor's degree in Nursing or related field required. Current RN license or RD certification required.

Experience: Three years of clinical experience in direct care of patients with diabetes and associated conditions. Equivalent combination of education and/or experience may be considered.

Certification/Registration/Licensure: RN Only: Must have current, valid RN license from the Texas Board of Nursing; or, valid Compact RN license. RD Only: Must be a Registered Dietitian by the Commission on Dietetic Registration and a Licensed RD On Dietitian by the Texas Department of Licensing and Regulation.

Skills or Special Abilities: Strong communication and interpersonal skills, with ability to work effectively with diverse populations. Bilingual (Spanish) language skills are highly desirable. Proficiency in electronic medical records and data analysis software is preferred.

Estimated Salary: $65,000 - $85,000 per year, based on experience and qualifications.
