Mental Health Connecticut, Inc. is seeking a highly skilled Community Outreach Case Manager to join our team.

About the Role

This full-time position will be responsible for providing direct services to adult individuals in recovery from chronic mental and co-occurring conditions. The successful candidate will work closely with clients to develop and implement Individualized Recovery Plans (IRPs) to improve their quality of life and maintain their highest possible ability to function in the community.

Key Responsibilities
  • Engage with clients to ensure they participate in programs and activities
  • Document all activities, communications, and encounters with clients according to service contract and MHC requirements
  • Ensure the safety of clients through proper supervision of assigned program site
  • Teach and assist Activities of Daily Living (ADL) skills to maintain a clean and orderly environment
  • Act as a liaison with therapists, referral sources, and community agencies
Requirements
  • GED or High School Diploma required
  • 2 years of related experience and/or degree in a related field, license, certification i.e. Recovery Assistant certificate
  • Knowledge of basic counseling skills
