Job Title: Nurse Practitioner

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at Nest Health. As a Nurse Practitioner, you will be responsible for providing high-quality, patient-centered care to our patients in the comfort of their own homes.

Job Summary:

The Nurse Practitioner will work as part of an interdisciplinary team to provide comprehensive primary and acute care to patients of all ages. This includes conducting thorough assessments, developing and implementing care plans, and collaborating with other healthcare professionals to ensure seamless care coordination.

Responsibilities:
  • Conduct thorough assessments of patients to identify their health needs and develop personalized care plans.
  • Provide routine and acute care to patients in their homes, including vaccinations, lab tests, and medication management.
  • Collaborate with other healthcare professionals, including physicians, nurses, and social workers, to ensure comprehensive care coordination.
  • Communicate effectively with patients and their families to educate them on their health conditions and treatment plans.
  • Document patient interactions and care plans in our electronic health record system.
Requirements:
  • Master's degree in Nursing with a focus on Nurse Practitioner studies.
  • Active, unrestricted license to practice as a Nurse Practitioner in Louisiana.
  • At least 2 years of experience providing clinical services to pediatric and adult patients with chronic medical and behavioral health conditions.
  • Proven skills and knowledge in clinical nursing skills, including blood drawing, patient assessments, and care plan reinforcement.
  • Ability to work independently and as part of an interdisciplinary team.
  • Availability to work 1-2 days per week, with Saturday availability preferred.
