We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Aveanna Healthcare, the largest pediatric home health care company in the U.S. As a LPN, you will provide skilled nursing care to patients in the home setting, working closely with our team to deliver high-quality care and support.

Job Responsibilities:
  • Provide and document skilled nursing care, under the supervision of a Registered Nurse, in accordance with the developed care plan and physicians orders.
  • Continuously observe and assess patient condition and care needs, reporting changes to the supervisor and/or physician as appropriate.
  • Document all activities, assessments, nursing actions, responses, and coordination of care in a timely manner.
  • Participate in and update the nursing care plan, taking appropriate nursing action based on assessment and achieving expected outcomes.
  • Recognize changes in patient needs and responses requiring intervention, implementing care to prevent risk or reduce risk.
Requirements:
  • Graduate of an accredited school of nursing.
  • Current, unrestricted state license as a Licensed Nurse in the state of practice.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
Preferred Qualifications:
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
  • Home health experience.
Physical Requirements:
  • Must be able to speak, write, read, and understand English.
  • Must be able to travel.
  • Must be able to lift 50 pounds.
  • Must be able to sufficiently reposition patients and move equipment without assistance.
