We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to provide high-quality patient care in the home setting. As a member of our team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Job Summary

The Licensed Practical Nurse (LPN) will be responsible for delivering and documenting skilled nursing care to patients in accordance with physician orders and developed care plans. This role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.

Responsibilities
  • Provide and document skilled nursing care to patients in the home setting
  • Develop and implement individualized care plans in collaboration with the healthcare team
  • Conduct regular assessments and evaluations to ensure patient safety and well-being
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination
  • Participate in ongoing education and training to maintain current knowledge and skills
Requirements
  • Graduate of an accredited school of nursing
  • Current, unrestricted state license as a Licensed Nurse in the state of practice
  • Current CPR certification
  • One year of experience in pediatrics or a related field (dependent on state requirements)
Preferred Qualifications
  • Six months of recent experience as a Licensed Nurse in a clinical care setting
  • Home health experience
Physical Requirements
  • Ability to speak, write, read, and understand English
  • Ability to travel
  • Ability to lift 50 pounds
  • Ability to sit and climb stairs
  • Visual and hearing acuity
  • Strong sense of smell and touch
