Job Summary

We are seeking a skilled and compassionate Home Health Nurse to join our team at Universal Pediatrics, Inc. As a Home Health Nurse, you will provide high-quality, patient-centered care to pediatric patients in their homes, promoting their health, safety, and well-being.

Key Responsibilities
  • Conduct thorough assessments, develop, and implement individualized care plans to meet the unique needs of each patient.
  • Administer medications, provide therapies, and interventions as scheduled in accordance with the plan of care and physician's orders.
  • Collaborate with the interdisciplinary team, including families, caregivers, and healthcare professionals, to ensure seamless care coordination and communication.
  • Provide education and support to patients and families to empower them to manage their care and make informed decisions.
  • Document patient progress, changes in condition, and other relevant information in a timely and accurate manner.
  • Participate in ongoing education and training to stay current with best practices, industry developments, and regulatory requirements.
Requirements
  • Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensure, without restrictions.
  • Valid driver's license and proof of auto liability insurance coverage.
  • CPR certification required, but may be obtained after hire.
  • COVID-19 vaccine compliance.
  • Ability to lift, carry, push, or pull up to 50 pounds.
