About the Role

We are seeking a skilled and compassionate Home Health Nurse to join our team at St. Luke's Health System. As a key member of our Home Health team, you will play a vital role in providing high-quality, patient-centered care to individuals in their homes.

Responsibilities
  • Provide comprehensive care to patients with a new diagnosis, medications, or have been discharged from ER, require respiratory treatment or wound care.
  • Ensure patients are progressing with their care plan, provide education related to their disease process, and assess physical and psychological needs to improve quality of life.
  • Coordinate resources and educate patient and patient's family during discharge.
What You'll Enjoy
  • Full-time days (Mon-Fri from 8:30a-5:00pm) with rotating evening call and weekends.
  • Daily visit requirements below state and national average.
  • Pay for drive time.
  • Mileage reimbursement (Geographical Area: Treasure Valley - Ada, Boise, Canyon, Gem, Owyhee, Payette, Washington).
  • Applicable Differential/On-call Pay.

Qualifications
  • Education: ASN or BSN Degree.
  • Experience: 1 year of RN experience; Hospice or Acute Care experience helpful.
  • Licenses/Certifications: Current RN License in the State of Practice; Current Basic Life Support (BLS) Certification.
  • Valid Driver's License and proof of Auto Insurance.
