About the Role

We are seeking a skilled Registered Nurse to join our team at Enhabit Home Health & Hospice. As a RN Case Manager, you will provide high-quality nursing care to patients in various senior living communities, including assisted living and independent living facilities.

Responsibilities
  • Supervise, evaluate, and provide nursing services to clients in senior living communities
  • Collaborate with facility wellness staff to ensure strong lines of communication and continuity of care
  • Consult with physicians and office staff as needed to ensure accurate patient care
  • Perform accurate OASIS collection, ensure medication profiles are current, and report lab values to physicians in a timely manner
  • Attend weekly case conferences and monthly case manager meetings
Qualifications
  • Current licensure as a Registered Nurse in the state of employment
  • Minimum of one year of clinical experience preferred
  • Demonstrate knowledge and skill in current nursing practice
  • Valid state driver's license and automobile liability insurance
