About the Role:
As a Home Health Registered Nurse with Providence, you will have the opportunity to provide high-quality, patient-centered care to individuals in the comfort of their own homes. Our team is dedicated to delivering compassionate and personalized care, and we are seeking a skilled and compassionate RN to join our team.
Responsibilities:
- Provide skilled nursing care to patients in their homes, including wound care, medication management, and education on disease management.
- Collaborate with the interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
- Conduct thorough assessments of patients' physical and emotional needs, and develop plans to address any identified gaps in care.
- Communicate effectively with patients, families, and caregivers to ensure that all parties are informed and engaged in the care process.
- Participate in ongoing education and training to stay current on best practices and evidence-based care.
Requirements:
- Current Oregon Registered Nurse License.
- 1 year of experience in medical/surgical care, preferably in an acute care setting.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
