Job SummaryWe are seeking a skilled Nurse Practitioner to join our team as a PRN/Part Time Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments in the comfort of patients' homes, identifying potential health risks and developing personalized care plans.
Key Responsibilities- Conduct adult/geriatric assessments, medical history, diagnosis, and treatment
- Collaborate with physicians, case managers, and other healthcare professionals to ensure seamless care coordination
- Provide education and support to patients and their families to promote healthy behaviors and disease management
- Document all assessments and care plans accurately and thoroughly
- Masters degree in nursing or equivalent experience
- Current RN and NP licensure in the state of practice, with prescription authority
- Board certification in a Matrix-approved specialty
- Minimum of 1 year experience as a Nurse Practitioner
- Strong computer skills and familiarity with electronic health records
