The Nurse Practitioner in Advanced Illness Management plays a vital role in facilitating tailored palliative care for our patients, ensuring that each treatment plan is comprehensive and individualized. This position requires exceptional clinical skills and ethical judgment, with responsibilities including diagnosis, treatment, and ongoing patient consultation under the guidance of the Palliative Medical Director or Primary Care physician.
Key Responsibilities
- Collaborate with the Professional Services Corporation (PSC) Physician and other palliative care team members to provide advanced illness management and palliative care, including diagnosis and follow-up for referred patients.
- Oversee the entire advanced illness management process, from initial assessment to discharge, addressing various factors such as disease progression, functional changes, and symptom management.
- Develop individualized care plans in partnership with patients, families, and the Advanced Illness team, including the Primary Care Physician.
- Facilitate access to necessary care as identified in the care plan.
- Participate in on-call services as required, delivering competent and responsive care.
- Prescribe medications and order diagnostic tests in accordance with state laws and professional practice standards.
- Maintain regular communication with the PSC Physician regarding clinical findings and care plan effectiveness.
- Foster effective relationships with the supportive care team and engage them as needed to achieve optimal patient outcomes.
- Document clinical findings and care provided in the patient's medical record, adhering to professional standards.
- Stay informed about Palliative Medicine Quality Measures and ensure accurate documentation and reporting.
Candidate Profile
Qualifications for Nurse Practitioners:
Essential Skills and Knowledge
- Strong leadership and communication skills, both written and verbal.
- Ability to make independent decisions and demonstrate self-motivation.
- Understanding of Palliative Medicine principles and relevant regulations.
- Compliance with continuing education requirements.
- Proficiency in electronic medical record systems.
Education and Experience
- Master's degree in Nursing from an accredited institution or equivalent as permitted by state law.
- A minimum of two years of experience as a Nurse Practitioner.
- Preferred specialties in Family or Adult Medicine.
- Experience in Value-Based Care practices is advantageous.
- Familiarity with Part B billing practices.
- Able to work independently in an Advanced Practice role.
Licensure and Certifications
- Active registered nurse license or equivalent as required by state law.
- Current Nurse Practitioner certification/license.
- Collaborative agreement with a supervising physician as mandated by state law.
- Part B Provider certification.
- Federal and state DEA licenses as required.
- ACHPN certification in Hospice and Palliative Care is preferred.
- Valid driver's license and automobile insurance.
