Outpatient Physical Therapy Specialist

POSITION OVERVIEW:

We are seeking a dedicated professional to expand our innovative service line focused on outpatient care within the comfort of patients' homes. This role operates on a per-visit compensation model, including rates for visits and training.

ROLE SUMMARY:

The Physical Therapist will deliver outpatient rehabilitation services directly at the patient's residence, which may include homes or assisted living facilities. Reliable transportation is essential for this position. The therapist will assess patients' functional abilities and implement tailored treatment plans aimed at enhancing or restoring their physical capabilities.

EDUCATIONAL REQUIREMENTS:

  • Must be a graduate from an accredited Physical Therapy program with at least a Bachelor’s degree.
  • A valid Connecticut Physical Therapy license is mandatory.
  • CPR certification is required.
  • A minimum of three years of recent experience in outpatient physical therapy is preferred.

SKILLS AND COMPETENCIES:

  • Strong verbal and written communication skills in English are essential.
  • Proficiency in using Windows-based software and personal computer applications is required.
  • Basic keyboarding skills and familiarity with Microsoft Office Suite (Outlook, Word, Excel) are advantageous.
  • Access to reliable transportation for travel between various patient locations is necessary.

KEY RESPONSIBILITIES:

  1. Conduct thorough evaluations of patients in accordance with departmental standards.
  2. Modify assessments based on individual patient symptoms, activity levels, and medical history, establishing specific, measurable goals.
  3. Implement appropriate therapeutic exercises to enhance strength, endurance, coordination, and range of motion.
  4. Utilize physical modalities and manual techniques to alleviate pain and achieve treatment objectives.
  5. Employ effective strategies to normalize motor functions and facilitate learning.
  6. Instruct patients and their families on home exercise regimens and functional activities to promote independence.
  7. Assess when patients have reached optimal benefits from therapy and recommend discharge when suitable.
  8. Maintain accurate documentation in compliance with insurance regulations and departmental policies.
  9. Foster communication with interdisciplinary teams and referring physicians regarding patient care.
  10. Ensure timely and accurate submission of billing information.
  11. Uphold the professional ethical standards set forth by the APTA and the facility.
  12. Practice safe body mechanics to prevent injuries during patient care.
  13. Engage in continuing education to enhance treatment methodologies.
  14. Participate in departmental or community educational initiatives as part of professional development.
