Work Schedule:

Monday to Friday, 8:00 AM - 4:30 PM, with 2-3 weekend shifts every 6 weeks.

AdventHealth East Orlando is a 265-bed community hospital that has been a leader in healthcare since 1941, addressing the needs of a rapidly growing population. Our recent expansion has enhanced our facilities, including a new patient tower and private rooms designed for a superior care experience.

Your Role:
The Clinical Social Services Manager will engage with patients facing complex psychosocial challenges, assisting them with eligibility for social programs and community resources. This role is essential in facilitating transitions of care and discharge planning for high-risk patients.
Key Responsibilities:
- Conduct psychosocial assessments and provide interventions.
- Receive referrals for patients with complex psychosocial needs from the healthcare team.
- Address issues related to child abuse, domestic violence, and other critical situations.
- Offer consultative services for patients lacking decision-making capacity, ensuring adherence to guardianship policies.
- Support foster care and adoption processes.
Qualifications Required:
- Master's degree in Social Work (MSW).
- A minimum of three years of experience in hospital or medical social work.
Preferred Qualifications:
- Experience in care management and discharge planning.
- Familiarity with state and federal care management guidelines.
- Licensed Clinical Social Worker (LCSW) certification.
