Overview

Southcoast Behavioral Health is a premier mental health treatment center in Dartmouth, Massachusetts, offering a comprehensive range of services for pediatric, adolescent, adult, and geriatric patients. Our team is dedicated to providing high-quality care and support to individuals and families affected by mental health and substance use disorders.

Purpose and Responsibilities

The Case Manager will play a critical role in managing patient cases with a high degree of clinical knowledge and expertise. Key responsibilities include:

  • Ensuring patients meet care criteria from admission through discharge
  • Initiating authorization with network providers
  • Maintaining knowledge of clinical information for patient stays by reviewing documentation, participating in staffing, attending care conferences, and collaborating with other treatment team members
  • Performing concurrent review with network providers and monitoring medical record documentation to promote compliance and ensure quality for retrospective review by external entities
  • Liaising with physicians and other treatment team members to ensure all aspects of treatment are fully documented
  • Leading the flow of information between levels of care by collaborating with admissions, clinical, and financial departments, as well as patients and their families/guardians, to provide a smooth transition at discharge
  • Coordinating resources with other community services and agencies
Qualifications

To be successful in this role, the ideal candidate will possess:

  • A Master's degree in Social Work, Counseling, or a related human service field
  • Licensure or certification for counseling, as required by the state in which the facility operates
  • CPR and de-escalation/restraint certification
  • Three or more years' experience working in a behavioral health setting
