About the Position

This is an exciting opportunity to join our team as a Nurse Practitioner or Physician Assistant. In this role, you will have the chance to make a meaningful difference in the lives of our patients and their families.

Key Responsibilities
  • Patient assessment and care planning: Conduct thorough assessments, develop treatment plans, and provide ongoing care management to residents.
  • Care coordination: Collaborate with healthcare teams to ensure seamless care transitions and optimize patient outcomes.
  • Patient education: Educate residents and their families on disease prevention, wellness, and self-management strategies.
Requirements
  • Nurse Practitioner or Physician Assistant licensure in good standing.
  • Masters degree in nursing or related field.
  • Excellent communication and interpersonal skills.

Estimated salary range: $125,000 - $160,000 per year, depending on experience and qualifications.
